I am aware that membership fees for S.I.C.A. are based on our volume of sales of sporting goods equipment in Canada. The following information is required for billing purposes only, and will remain confidential.

**The U.S. Hockey Group requires a seperate membership; therefore, for additional information on our fees, please contact S.I.C.A.

In accordance with the guidelines listed below and based on our last fiscal year sales, our Company qualifies in the following category:

0 to 1 million in sales

1 to 3 million in sales

3 to 5 million in sales

5 to 25 million in sales

25 to 50 million in sales

50 million in sales and over

	Guidelines
	Calculation of Sporting Goods Sales in Canada shall include:
		a) All hard goods and accessories sold to all outlets.
		b) All soft goods generally considered to be athletic wear which is sold to sporting goods outlets or sporting goods sections of department stores.
	Associated of Related companies and divisions:
		For the purpose of computation, where a company having more than one(1) division maintains one(1) Credit Department, the volume of sales under the supervision of that Credit Department shall be combined. Where a company having more than one(1) division maintains separate Credit Departments, the volume of sales supervised by each Credit Department shall be stated separately.

	All applications are subject to approval by the Board of Directors.
